Fees, Payments, Privacy, & Policies

Full payment is due at the time of service. Currently, we are private pay only (we do not accept insurance).  We accept Visa, MasterCard, Discover, American Express, personal checks, and cash.  Checks returned for insufficient funds will be subject to a $25 insufficient funds fee (NSF).

Your appointment time is especially reserved just for you, and we are not otherwise reimbursed for the costs associated with missed or cancelled appointments.  As a result, appointments cancelled with less than 24-hours notice will be subject to a $75 Late Cancellation Fee. Missed appointments or appointments cancelled after the start time will be subject to a $150 Missed Appointment Fee.

Your privacy is important to us, so it's important to know that there may be times we are legally required to break confidentiality without your consent (e.g., if a person's safety becomes a concern, to comply with law, to protect children or adults from abuse, for subpoenas, etc.).  Click the button below to download a copy of our Notice of Privacy Practices.
